Chronic Conditions Education and Management

Chronic conditions are medical conditions that are expected to last longer than 6 months and in most cases are life long and although they are managble they are often not curable and may progress over time.

Chronic conditions may be a result of lifestyle factors (nutrition, physical activity, smoking, drugs and alcohol), genetics, autoimmune, occupational/recreational exposure (chemicals/poisons/radiation/etc), trauma or surgical complications.

Most chronic conditions are managed through lifestyle changes, medications and non pharmocolgical therapies (massage/TENS/heat/hydro) and often monitored through medical imaging or pathology investigations.

 

Chronic condition education and person centered care planning  is vital for patients managing their chronic conditions as the patient is responsible for managing their chronic condition in collaboration with their healthcare team (not the other way round).

Healthy Ageing

Ageing in unavaoidable and every measure should be taken to ensure elderly people can live long and safe lives in their own homes as long as possible. In some cases people may need to downsize their home, move into a retirement villiage or a residential aged care facility. 

Healthy ageing is a journey, not a destination and requires ongoing adaption to physical, mental/emotional, financial and social changes as people age and ensuring they have made plans for end of life care and affairs. 

Performing regular health checks, screening and colllaboration with your healthcare team will improve your healthy ageing journey experience, quality of life and provide early detection of changes in health and healthcare needs.


Healthy ageing does not look or feel the same for everyone and should always be person centred where appropriate and discussed or explained where not appropriate.
